Request Medical Records

For a copy of your medical records:

1. Please click the link below to submit a medical records request; Do NOT submit your request in person.

2. There will be a $6.50 charge for your medical records.

3. ATTENION: If you need imaging (e.g. MRI, X-Ray, CT Scan) in addition to medical records, this will need to be requested separately. Please go to https://nhoc.com/images/ or email medicalrecords@nhoc.com.

4. You should receive your records within 5-7 business days of completing payment for your request.
